A Clinical Study on the Laproscopic Tubal Sterilization

Abstract
recently the increased interest in population control as well as concern over the safety and effectiveness of other contraception have resulted in a marked increase in the demand for frmale sterilization that are safe and effective and can be done with mininal expences and hospitalization. In this regard, laparoscopic tubal sterilization which has advantages of a minimal single infraumbilical incision, use of local anesthesia, and simplified instrumentation has become a supreme method of outpatient tubal steriliszation has become a supreme method of outpatient tubal sterilization throughout the world. From september 1976 to November 1977, of 250 cases of laparoscopic tubal sterilization, 100 procedures of silastic band application and 150 procedures of electro coagulation by randome selection were attempted to observe the nature and frequency of side effects and complications as well as the postoperative failure at the Seoul National University Hospital. Comparison in various aspects was also made between silastic band technique and electro coagulation proceure. The obtained results were as follows; 1. Average age of client was 34.4 year, the youngest was 25 years, and the oldest was 49 years. About 41.6 percent of clients aged 30-34 2. Women having 3 living children represented 48 percent of the total and average number of living children was 2.8. 3. Only 0.8 percent of the cases had not experienced indueced abortion, where as about 6.4 percent of clients had experienced in 7 or more induced abortion, The greatest number of induced abortion was 15. 4. According to the sex of their children, the greatest number of cases had 2 sons and 1 daughter. About 44.8 percent of cases had 2 sons, while 48.0 percent of the clients had only one daughter. Their average number of sons was 1.7, and the average number of daughter was 1.5, which showed a trend of male preference. 5. About 81.2 % of the clients had experienced in induced abortion before this sterilization compared with 18.0% havingterm deliveries. Educational level showed that graduate of secondary school rated first comprising 63.2% of the total and college graduates rated 21.5%. Comparatively higher educated women were more accepting of this procedure. 6. Contraceptive methods priorto this sterilization were 32.4% with oral contraceptives, 31.6 percent of the cases had not used any contraceptives. 20.0 percent were condom users and IUD was 13.2 percent. 7. Sterilization procedures have been performed under local anesthesia in 97.2 percent and under general anesthesia in 2.8 percent. 8. The operative time was less than 15 minutes in about 90.0 percent in both procedures. 9. Seventy nine percent of the silastic band group had been hospitalized for 3 hours or less whereas 63.3 percent of the electro-coagulation group, for 3 hours or less. 10. Among the total number of 250 cases, 18 women had a previous history of abdominal surgery, the majority of this being appendectmy and cesarean section. And no specific difficulty was encountered during the tubal sterilization. 11. The incidental pregnancy termination in the first trimester and laparoscopic sterilization were performen in 83 cases out of the total and there was no significant increase in operative time and complication, which compared with those in nonpregnant women.
